There's one in Messaien's Turangalia Symphony. I haven't listened to that in a while. I think I will tonight.

It has both a slider and piano keys. Pedals too, I think those are just for volume control, but I'm not sure since there's two of them. Sustain, maybe? I have no idea what the lute-like thing on the speaker does.
Here's a pic from Wikipedia. It's hard to see the slider but you can just barely see the brown "knob" right in front of the keyboard.

I think the Beach Boys actually did use a Theremin in the recording studio, but live they used an "electro-theremin" (LANK), which is like the Ondes Martenot but with just the sliding mechanism and no piano style keys.
